As in "I'm sure as hell not going to let you" sure as hell seems to have been started by people for whom hell was a fact beyond question. Of course the literal meaning of the phrase is the opposite of how it is used but there is no way in hell I am going to stop using it just because it is nonsensical. And no, I don't have any thoughts on that one yet.
